Cryoglobulin deposits on a blood smear.

چکیده
A 61-year-old male presented with 2 months of bone pain. Complete blood count showed Hb 10 g/dL, white blood cell count 4.5 109/L, and platelets 130 109/L. The peripheral smear contained visible agglutinates and a blue background color (panel A). Aggregates of amorphous particles mimicking lysed cells were observed microscopically (panels C-E) with smaller and less stained deposits (panel E arrow), some resembling platelets (panel E stars). These findings suggested cryoglobulins. After warming the blood (37°C 4 hours), the smear regained its usual staining characteristics (panel B) and was free of microscopic deposits. A serum prepared at 37°C demonstrated protein precipitation when cooled to room temperature. This was determined to be an IgG, monoclonal protein (29.3 g/L). Afterward, the patient was noted to have sternal and vertebral lytic lesions. Bone marrow examination revealed 34% plasma cells, and a diagnosis of mutliple myeloma was made.

No adenopathy, neither liver or spleen enlargeme...
متن کاملDetection of cryoglobulins in serum of Caspian miniature horse
Blood samples were collected from 20 healthy miniature Caspian horses at 37 °C. Isolation of cryoglobulin was performed based on a standard method in present study. Harvested sera were kept at 4 °C for two hours and then examined for cryoglubolin. Four serum samples containing precipitate Suspicious of containing cryoglobulin were selected. Subsequently serum protein electrophoresis was perform...

INTRODUCTION Laboratory tests play a central role in assessing a patient and orienting the diagnostic evaluation. We report a case where the discrepancy between the manual and automatic cell count gave a hint to the final diagnosis. CASE PRESENTATION A 55-year-old Caucasian man, known to have hepatitis C, was admitted with acute respiratory failure secondary to acute pulmonary edema and diffu...
متن کاملA Trial of Complement Inhibition in a Patient with Cryoglobulin-Induced Glomerulonephritis
Cryoglobulinemia induces an immune complex-mediated glomerulonephritis that is characterized by the presence of large immune deposits, including complement C3 and C5b-9, marked macrophage influx and mesangial cell proliferation. The precise role of complement in cryoglobulin-induced glomerulonephritis in humans remains unclear, whereas in mice there has been evidence that complement activation ...
متن کاملDevice for carrying blood samples at 37°C for cryoglobulin test.
To reliably transport blood samples for cryoglobulin analysis, we have created a sample transport device containing a mixture of two waxes that solidifies at 38°C and maintains sample temperature at 38°C. Samples arriving at the laboratory at 37 to 38°C increased to 95% from 34% with the use of the device.
